Collection Info:
1. Proper collection is vital to ensure the optimal specimen
2. Masturbation is the ONLY acceptable method of specimen collection. The entire ejaculate must be collected into the plastic cup
provided. Latex and lubricants are toxic to sperm.
3. The optimal place for collection is at the Laboratory
4. Sample needs to arrive at the laboratory 30 minutes post collection and kept at body temperature.
5. Samples should be collected after a minimum of 2 days and a maximum of 7 days of sexual abstinence.
6. Make sure your name, date of birth, date and time of collection are written on the cup.
7. A brief questionnaire needs to be filled out at the laboratory, or can be printed off the website and accompany the sample.
8. You will also need a copy of the Dr.’s orders

Infertility investigation. Includes volume, viscosity, pH, motility, count, normal forms, by Kruger-Strict criteria, and total # of sperm and motile sperm. If a zero sperm count is obtained, a "Semen fructose, semen" will automatically be ordered and performed.
